Terms and Conditions
1) Payment is required up front and must include any additional fees or charges. Checks are accepted at this time. Client understands this contract and takes responsibility for prompt payment of fees. Client authorizes this signed contract to be valid approval for future services of any purpose provided by this contract permitting Tango Pets LLC to accept telephone, email, or via web reservations for services and enter premises without any additional signed contracts or written authorization.
2) Client recognizes that Tango Pets LLC is not responsible for any unintentional errors, omissions, or incorrect assertions. Further, the Client will remain responsible for the actions of his or her dog at all times and hereby agree to indemnify and hold Tango Pets LLC harmless of any and all claims of injury, bites, expense, costs, or damages caused by the actions of my dog while under Tango Pets LLC care. If a bite occurs, Tango Pets LLC is responsible for reporting the incident to authorities. Additionally, Tango Pets LLC will act with caution in my home in my absence and I hereby agree to indemnify and hold harmless Tango Pets LLC of any and all claims of damages to my home.
3) Tango Pets LLC is authorized to walk or pet sit the dog as outlined in this contract. I agree
to the use of humane equipment such as head-collars or specialized body harness. Tango Pets LLC will not use traditional “training” equipment such as choke chains, prong collars or shock collars.
4) Tango Pets LLC is also authorized to seek any medical care if deemed necessary with
release from all liabilities related to transportation, treatment and expenses. Tango Pets LLC is authorized to approve medical and/or emergency treatment (excluding euthanasia) as recommended by a veterinarian. I agree to reimburse Tango Pets LLC for any expenses incurred attending to this need
5) In the event of personal emergencies or illnesses, Tango Pets LLC will notify client as promptly as possible and compensation will be given to client
6) All dogs are required to be fully vaccinated and be licensed by their city.
7) Tango Pets LLC reserves the right to terminate this contract at any time before or during
this term.
8) Client understands that this contract is for walking or pet sitting only.
Release of Liability
I do hereby waive and release Tango Pets LLC from any and all liabilities of any nature for the
actions of myself, my pet(s), or any other person who accompanies me, or holds a key to my home; except those arising from negligence or willful misconduct on the part of Tango Pets LLC. Tango Pets LLC agrees to provide all services in kind, humane, reliable and trustworthy manner. Client agrees to notify Tango Pets LLC of any concerns within 24 hours of their return. In case of an emergency, inclement weather or a natural disaster I authorize Tango Pets LLC to use their reasonable judgment for the care and well being of my pet(s) and/or house. I understand that Tango Pets LLC can terminate this contract if my pet becomes a threat to the safety or health of Tango Pets LLC or the community due to aggressive behavior. Tango Pets LLC will contact client if pet(s) displays aggressive behaviors or tendencies and an educational forum can be arranged. Tango Pets LLC specializes in various forms of dog aggression and will do what we can to help families with special dog needs. Tango Pets LLC is required to report any bite incidents to authorities. I acknowledge I am responsible for medical expenses and damages resulting from an injury to Tango Pets LLC employees or other person or animal caused by my pet(s). Tango Pets LLC reserves the right to refuse service to any client, at any time, for any reason.
I attest that all of the above information is true to the best of my knowledge. If anything changes from what is listed, I will inform Tango Pets LLC before the next service is scheduled. This signed document gives Tango Pets LLC authorization to enter the above listed address as needed to perform the necessary services. I authorize this contract to be a valid approval to permit Tango Pets LLC to accept all future telephone, email, mail, on-line reservations and enter my home without additional signed contracts or written authorizations.
Cancellation Policy
We know that circumstances arise that may cause you to cancel. We're flexible, but please try to call before 9:00am. If failed to notify before 9:00am, you may be required to pay for service.
Inclement Weather Policy
Weather conditions should not impact a dog’s physical health or well-being.
In the event of inclement weather, the client can agree to a home visit by Tango Pets LLC.
The dog will be given a quick walk and then the rest of the time will be in-doors.
House Keys
2 copies of your house keys are required. One copy is for daily use – the other is kept in a secure location as a back-up in case of emergency. All keys are coded and do not have your name or address written on them. If you do not have 2 copies ready for us at the pre-service appointment, we can make a copy for you for $5.
During my absence, Tango Pets LLC, will be caring for my pet(s) and has my permission to transport my pet(s) for treatment.
I give permission to Tango Pets LLC to approve treatment up to the amount indicated.
I will assume full responsibility upon my return for payment and/or reimbursement for veterinary services rendered up to the above stated amount.
If the veterinary office name above is unavailable, or in case of an emergency, if the location is too far, I authorize Tango Pets LLC to take my pet(s) to another veterinary office for treatment I understand that Tango Pets LLC assumes no responsibility for the loss of my pet(s) and is released from all liability related to transportation, treatment and expense.
This consent for treatment has no expiration date unless otherwise noted.
